Adult Day Services
Adult Day Services provide a
daytime structured program of social and rehabilitative and/or
maintenance services to functionally impaired elderly persons.
Most sites will accept clients from anywhere in the area.
Limited transportation to and from sites may be available. Some
of these programs also offer dementia specific services. Contact

Caregiver Support
The organizations listed have
received funds to provide training for caregivers and/or support
groups. Local hospitals and health-based associations (i.e.,
Parkinson Foundation) often have caregiving classes and support
groups. Call them directly for more information.

Caregiver services include: support and education groups,
caregiver series, individual caregiver support, educational
presentations, information tables and caregiving conferences in
western Wayne County. Series are offered at community sites, for
family and friends caring for older adults. Topics include:
responding to failing health, normal changes in aging, memory
loss, and communication, handling stress, the emotions of
caregiving, coping strategies, and community resources. Support
and education groups are available for: husbands and wives
caring for ill spouses, families caring for persons with memory
loss and persons caring for older adults. Monthly support groups
for caregivers provide an opportunity to share information,
ideas, and resources as they help and encourage one another.
Caregiver contacts are provided in person, on the phone and by
email. Information tables are available for community meetings,
employee lunch hours, faith based programs and other events.

The Caregiver Training and Support Classes will address the
needs of caregivers and educate them on options and resources
that can help provide relief. The training series is combined
with support groups that offer an opportunity for caregivers to
address problems and find solutions to caregiving challenges.
The caregiver meetings have morning and afternoon weekly
sessions. Participants are welcome to attend all sessions or
choose specific meetings.

Wayne-Metro provides the Senior Services Case Management Program
for at-risk seniors and families. The program offers support and
resources for family caregivers and helps to minimize health and
safety risks in the home. The program has been designed to meet
the common needs of families who utilize Adult Day Services, as
well as individuals and families throughout western and southern

Chore Referral
Seniors are given the names of at least
three independent workers (when available) who have been asked
to charge reasonable fees. Seniors make their own arrangements
for the work and payment for service. Generally the most common
types of assistance requested are: lawn mowing, snow removal,
leaf raking, light housekeeping, or minor home repairs. Many
Senior Centers also maintain a referral list for services.

Friendly Reassurance
This program provides for regular telephone
contacts or home visits with homebound older persons to assure
their well-being and safety and to provide social interaction.
Calls are usually made Monday - Friday. Reassurance programs may
also be called Telephone Reassurance or Tele-Care program.
Many communities not listed below operate their own Reassurance
program. Contact your local senior center for more information.

Gatekeeper Program
The Gatekeeper program is designed
to assist frail, isolated and vulnerable older adults who have
little contact with others and who may go unnoticed when they
need attention. As an example, a utility company worker who
notices a senior in distress will report the matter to the
Gatekeeper contact in their office. A referral would then be
made to The Senior Alliance.

Geriatric Assessment
A geriatric assessment is
completed by a multi-disciplinary team specializing in geriatric
medicine. It will consist of a physician, nurse, social worker
and may include a psychiatrist, dietician or pharmacist. This
team will assess the older adult=s physical and mental health
status including problems faced by the individual and their
family. Recommendations will be made regarding a treatment plan,
educational information, referrals to community resources, and
other supportive services needed

Statewide collaborative agency dedicated to improving the
quality of life for persons who are deaf, hard of hearing, and

Homeless Assistance
Wayne-Metropolitan Community
Action Agency, in partnership with The Salvation Army, Travelers
Aid Society, and the Detroit-Wayne County Community Mental
Health Agency, has formed the Out-Wayne County Homeless
Coalition.
Services include: temporary shelter for families and
individuals, financial assistance, educational and training
programs, case management support services and transportation.

Kinship Caregiver Support
Kinship care is the full time
care, nurturing and protection of children by relatives, members
of their tribes or clans, godparents, stepparents, or any adult
who has a kinship bond with a child ~ Child Welfare League of
America. Programs may also be called "Grandparents Raising
Grandchildren."

Grandparents as Caregivers Training Program offers workshops
addressing the caregiving concerns of grandparents who have the
primary caregiving responsibilities for their grandchildren.
Services include information and referral, periodic educational
series on the issues grandparents face, and monthly support
groups for grandparents. An AWBS booklet entitled AGrandparents
Guide to Raising Grandchildren@ for Wayne County can lead
grandparents through many of the common challenges such as
finding help with expenses, food, healthcare and legal advice.

Long Term Care Ombudsman
The Long Term Care (LTC) Ombudsman
provides assistance to residents of LTC facilities by helping to
resolve complaints; provide information about rights and rules;
and referrals to community resources. Assistance to prospective
LTC residents and their families regarding placement, financing,
and options. Citizens for Better Care (CBC) utilizes their staff
and trained volunteers to help residents of nursing homes, Adult
Foster Care homes, Homes for the Aged address concerns with
their care and housing. They also provide information about LTC
options, including lists of facilities. CBC will also provide
licensing reports on nursing homes, Homes for the Aged, and
Adult Foster Care homes.

Personal Emergency Response System
The Personal Emergency Response
System (PERS) is connected to a phone line to provide a fast,
easy connection to a 24 hour response center. The subscriber
would wear a button pendant or wrist band to access assistance.
There is an installation fee and monthly service charge. Many
companies also offer medication reminders and devices to provide
support in caring for someone with dementia. Different models
and services are available, discuss the various options and cost
with the companies. Inclusion in this list does not imply
