Home Care services are designed to assist consumers to remain in
their own homes and function as independently as possible. Even
though resources to provide relief for caregivers have increased
in recent years, in most cases, caregiving is provided by family
members. The Senior Alliance’s Community Care Department
oversees the Care Management, Case Coordination & Support, and
MI Choice Waiver programs. The Department of Human Services also
provides home care assistance (Adult Home Help program) to
Medicaid qualified individuals.

Personal Care, Homemaking & Respite Care
Homemaker service includes
assistance with tasks such as laundry, ironing, meal
preparation, shopping for necessities, and light housekeeping
(dusting, vacuuming, mopping floors, cleaning bathroom and
kitchen, and making beds). Personal Care is designed to assist
clients with tasks such as bathing, dressing, grooming,
toileting, transferring, eating, and ambulation. Respite
Care provides relief to
caregivers at home. The service includes attendant care
(assistance with toileting, eating, and ambulation), and basic
care (assistance with activities of daily living, routine
exercise regimen, and supervision of adherence to medication).

Community Care Department as listed below.

telephone screening.

Care Management
Care Management (CM) is designed
to help the frail older person who wants to remain at home.
Generally, these seniors have needs that are more complex than
those requiring assistance through the CCS program. After a
preliminary qualification screening is completed over the phone,
an in-home assessment is conducted by a care manager, who is a
registered nurse or social worker. Supportive services provided
are primarily personal care, homemaker, and respite care.
The Care Manager will:

Meet with the client in
his/her home to learn about the client's abilities and
needs.

Work with the client and the family to design a plan of
care to meet those needs.

Arrange for services from health and social agencies, as
well as family and friends, to meet the client's needs.

Remain in close contact with the client, their family, and
service providers, adjusting services as needed.

To qualify for Care Management, individuals must meet one
of the following criteria:
-
Have multiple and complex needs or problems.
-
Be unable to take care of his/her needs; and family and
friends are unable to provide all of the care that is
needed.

Is currently in an institution and no longer needs all the
services available there, but needs help to return home.
-
Is currently living at home and considering a move to
another living arrangement.

Case Coordination & Support
Case Coordination & Support (CCS)
is designed to help seniors live as independently as possible,
by putting in-home support services in place. Seniors served
through the CCS program are generally not as frail as those
served through the Care Management program. Care managers
conduct in-home assessments of potential clients to determine
eligibility for services such as Homemaker, Personal Care, and
Respite Care. After eligibility has been determined, and a care
plan developed, The Senior Alliance will purchase services from
local and regional providers.

MI Choice is a home and
community-based program. Eligible adults must meet financial
requirements and are approved medically appropriate for nursing
home care to be a MI Choice participant. This program allows the
individual to access services in the community along with
receiving support in their own home or other residential
setting. Each participant can obtain basic Medicaid-covered
services and must receive one or more MI Choice Waiver services.
The program includes prescreening, assessments, case management,
direct purchase of services, and re-assessment contacts. MI
Choice Waiver is a statewide program funded by the Michigan
Department of Community Health (MDCH). Eligibility Requirements:
Age
Financial
To be financially eligible for MI Choice, participants must
qualify for Medicaid using special income and asset rules. The
income limit is 300% of Supplemental Security Income (SSI)
allotment set by the federal government each year. Persons
currently receiving SSI benefits or Medicaid services meet the
program financial criteria.

Participant’s individual
income up to $1,869 per month for 2007. Amount subject
to change. If the client has a spouse, the spouse’s
income is not computed in the qualifying amount.
-
Allowable assets for an individual client are limited
to $2,000. If the client has a spouse living in the
community, the federal Protected Spousal Asset
guidelines are utilized. Currently, the Protected
Spousal Asset is one half of the couple’s countable
assets – no less than $20,328 and not to exceed
$101,640. Amounts are subject to change.
Medical
Medical status must meet the Michigan Medicaid nursing facility
level of care determination criteria.
Both agencies listed below operate the MI Choice
Waiver program in western and southern Wayne County
under the same State guidelines.

Nursing Facility Transition Services
Nursing Facility Transition
Services are available to assist persons receiving long term
care in transitioning from a Nursing Facility into the
community. Based on the individual program participant’s needs,
available services may include but are not limited to:
Coordination and Support, Housing assistance, Furniture for
participant’s home, Home modifications, Groceries, Moving
expenses, Utility deposits/hook-up fees, and Household goods.
Other items required for a successful transition from a facility
to a community based environment may also be available.
Nursing Facility Transition Services are funded by the Michigan
Department of Community Health (MDCH) and offered statewide.
Eligibility: Persons must reside in a nursing
facility and be eligible for Medicaid to obtain Nursing Facility
Transition Services. Ongoing assistance in the community may be
available based on separate eligibility criteria. Program
participants may be eligible for ongoing services through the MI
Choice Waiver Program.

Adult Home Help Program
The Department of Human Services
(DHS) assists functionally limited individuals who are Medicaid
eligible to remain in their independent settings for as long as
possible. Assistance is provided for the following activities:
personal care, laundry, housework, shopping & errands.

Help Provider Registry” for people who qualify for the DHS Adult
Home Help Program. The Registry includes names of individuals

Center for Senior Independence
The Center for Senior Independence
is a Program of All-Inclusive Care for the Elderly (PACE)
program managed by the Henry Ford Health System. The Center
provides an alternative to nursing home care for older adults by
allowing participants to remain in their own homes and
communities. The array of services provides respite and
assistance to caregivers.
The program features a comprehensive medical and social service
delivery system using an interdisciplinary team approach in an
adult day health center that is supplemented by in-home and
referral services in accordance with participants’ needs.
Transportation is provided to and from the Center and to
specialized appointments.
Participants must be 55 years of age or older who require
long-term medical and supportive services (certified as eligible
for nursing home care) and a resident of Redford, Detroit,
Highland Park, Hamtramck, Harper Woods, or the Grosse Pointes.

Senior Companion Program
The Senior Companion Program of
Wayne County provides volunteer opportunities for older adults
who can provide companionship on a part-time basis to homebound
seniors. Typically, a senior companion can provide several
tasks. These may include: companionship, assistance with minor
household tasks, light grocery shopping, meal preparation,
respite services, accompaniment to medical appointments,
advocacy and support, assistance with physical therapy and/or
daily exercises, personal grooming, etc. To qualify to receive
senior companion services, clients must be age 60 or older and
be identified with one or more of the following problems:
homebound, socially isolated, recuperating from surgery, illness
or accident, emotionally impaired, developmentally disabled, or
a caregiver who desires respite services. There is no fee for
this service which is funded by the Michigan Office of Services
to the Aging.

Home Health Agencies
These agencies offer home care
services. Individuals can make private pay arrangements directly
with service providers. Even though some businesses are located
outside of our service area, they provide service in western &
southern Wayne County unless otherwise noted.
Medicare offers Home Health Compare at
www.medicare.gov. It is an online tool to obtain information
about Medicare-certified home health agencies.
Some companies provide private-duty
services only and do not offer skilled Medicare services.
When deciding on a home care agency, confirm they have
insurance and complete background checks on their staff.
The Michigan Office of Services to
the Aging has a “Caregiver Hiring Guide” posted on their website
at www.michigan.gov/miseniors.
Medicare Coverage of Skilled Home Health Care
The Medicare program will pay for home health care if all of the
following conditions are met:

Your doctor must decide
that you need medical care at home, and make a plan for
your care at home
-
You must need at least one of the following:
intermittent skilled nursing care, or physical therapy,
or speech-language therapy, or continue to need
occupational therapy
-
You must be homebound, or normally unable to leave
home unassisted. To be homebound means that leaving home
takes considerable and taxing effort. A person may leave
home for medical treatment or short, infrequent absences
for non-medical reasons, such as a trip to the barber or
to attend religious services. A need for adult day
services does not keep you from getting home health care
for other medical conditions, and

The home health care agency caring for you must be
Medicare certified.
Contact your physician if you feel
you may qualify for skilled care benefits.

Hospice Care
Hospice care is described as "a
team-oriented approach of expert medical care, pain management,
and emotional and spiritual support expressly tailored to the
patient's wishes. Emotional and spiritual support also is
extended to the family and loved ones. Generally, this care is
provided in the patient's home or in a home-like setting
operated by a hospice program. Medicare, private health
insurance, and Medicaid in most states cover hospice care for
patients who meet certain criteria." - National Hospice
Organization.
Hospice services are available in your home, apartment,
outpatient, hospital, licensed assisted living and nursing home
facilities. Some hospice organizations manage licensed
in-patient care residences which are listed under "Licensed Care
Housing & Facilities" in this directory.
