Long Term Care Connections, the single point of entry demonstration projects, serve as a coordinated system of information and assistance in planning for and accessing long term care services. The single points of entry minimize confusion, enhance individual choice, and support informed consumer decision-making. Options Counseling is offered to improve an individual’s understanding of the array of long term care supports that are available using Person Centered Planning.

The Detroit/Wayne County Long Term Care Connection (DWCLTCC) is currently serving as the single point of entry for Public Service Area 1-A.

Long Term Care Information & Assistance in the 34 communities of southern and western Wayne County (PSA 1-C) is currently available. Consumers may call 866-642-4582 for assistance.

DWCTLCC is a demonstration project funded by grants for the Administration on Aging (AoA) and The Centers for Medicare and Medicaid (CMS). An Aging and Disability Resource Center (ADRC) grant was awarded to Michigan on July 1, 2006.

Why the Need?
A recent Mathematic study found that most Americans do not begin to explore Long Term Care (LTC) options until the need is urgent. Also, 71% of those studied do not understand the cost involved with LTC nor know where to go to attain information.

In 2003 about 3.1 million older persons and persons with disabilities received Medicaid financed long term care. Just over half (55%) were living in nursing homes. Total combined Medicaid and Medicare expenditures for this population were approximately $132 billion.

Currently, there is no single authoritative source of information on long term care in Michigan to aid consumers in LTC decisions.

Federal grants to fund the Single Point of Entry have been awarded to 43 states since 2003 including Michigan. 

How will it work?
Consumers in need of Long Term Care Information & Assistance can call 866-642-4582 or locally (734) 727-2088 and will be connected with an Information and Assistance expert in their community.

Information that is accurate and pertinent to the request of the consumer is provided including supports, services and planning options.

The Single Point of Entry program is consumer driven. All information and assistance services are FREE.

What is the role of The Senior Alliance?
The Senior Alliance (TSA) is responsible for a portion of western and southern Wayne County’s phase-in of this demonstration project.

TSA is partnered with the Detroit Wayne County Long Term Care Connection and is responsible for providing specialized Long Term Care Information & Assistance.

How can you become involved?
Periodic informational meetings are scheduled locally in western and southern Wayne County and are open to public.

Also, The Michigan Department of Community Health and The Office of Long Term Care Supports and Services host public bi-monthly stakeholders meetings in Lansing.
